Cascade Health COVID Vaccination Scheduler
Cascade Health will be providing weekly COVID vaccination clinics for employers in Lane County. Our first clinic will start on Tuesday March 30th (please see information below and attached). We will have a clinic every Tuesday thereafter for probably 3-4 weeks depending on demand. We are relying on the honor system for those who sign up to be in a current priority group.

COVID Vaccination Clinic
Cascade Health will be providing a first dose COVID vaccination clinic on Tuesday, March 30th. Signup is a first come, first serve basis. This clinic is first doses only for currently approved priority groups. Please see the attached information about priority groups.
Please click the link below to register for your appointment time. You will need a valid email address to register. You may not utilize the same email address to register for multiple individuals. We will be contacting you to schedule your second dose via this email address. There will be a 15-30 min observation period after you receive your vaccine. Please arrive at your scheduled appointment time.
Please complete the attached consent form. You will need to bring this completed form to your appointment. This is your voucher to receive the vaccine. If you forget to bring this form or if it is incomplete, this will delay your appointment time. Please make sure to bring your insurance card and fill in the insurance information on the consent form. While there is no charge for the vaccine, we need to document the information. If you are uninsured, there will be no charge for the vaccine. Please contact your employer with any questions.
